Avenues to Independence
Avenues to Independence provides a variety of flexible day programming options located in the northwest suburbs and Chicago. These include contract packaging jobs, community employment, workforce training, computer training, art programming, and entrepreneurial opportunities.
Avenues develops employment opportunities, on-the-job training and support services for persons employed in the community. We provide job searching, application and interviewing skills, and job coaches on a limited basis. Our Community Living Facility (CLF) offers adults an opportunity for semi-independent living with staff members on the premises 24 hours a day to assist and train individuals in various daily living skills.
Most of Avenues' Community Integrated Living Arrangement (CILA) homes provide 24 hour staff on premises support. The men and women living in these homes received personalized support from dedicated staff.

Blue Island Citizens for People With Developmental Disabilities

Senior Program offers older individuals living with intellectual and Developmental Disabilities the opportunity to retain their current life skills while embracing their golden years.
Blended Approach Participation in multiple programs is part of a "Blended Day." A blended day is an approach to creating an open environment where people can choose which activities they would like to do for the day. For example, someone can spend half a day volunteering and the other half in recreational activities.
Residential offers a variety of supportive living arrangements in the community. People live in their community with staff support for all aspects of daily living, caring for their home, and interacting and participating in their community. Residential services basically supports life.

Home-Based Services offers a facilitator to support families receiving services for children and adults with intellectual and Developmental Disabilities who are living at home. Service Facilitation includes case management services to assist individuals and families in gaining access to needed Waiver and other Medicaid State Plan services, medical, social, educational and other services. Service Facilitators are responsible for the day-to-day oversight and administration of the service plan and for ensuring the health, safety and welfare of the individual.

CRA provides community based housing and support services to individuals with
developmental and intellectual disabilities. Twenty-Four Hour CILA
These homes have on-site shift staff available during all times when participants
are present. Staff provide both scheduled and unscheduled supports and services
as needed by participants.
Services provided include Intermittent CILA, which serves individuals whose
support plan documents that they do not require 24-hour, on-site staff presence
and documents the situations in which individuals may safely be away from direct
staff supervision. Intermittent Residential services in apartments or family homes
have staff available on call 24 hours per day. On-site shift staff are available to provide
both scheduled and unscheduled supports and services as needed by the participants
served and as specified in each participant's support plan. Additional services include
Family CILA, which is Similar to Intermittent CILA, except that individuals are provided
support in the home of their natural family/relatives and/or friends, Person Centered
Planning, where each individual will have a personalized centered support plan developed

community Integration where Individuals will be provided ongoing participation in community
life and time spent out of the living arrangement for events such as religious, Civic, education,
library, clubs, shopping and amusement. Other such events include participation in family activities

CIRCLE OF SUPPORT INC
Circle of Support, Inc. offers Family CILA, Intermittent CILA, Adult and Children Home- Based Waiver programs which are funded by the IL Department of Human
Services, Division of Developmental Disabilities.
We provide services to both children and adults through the Illinois Department of Human Services and through private customized services. Our services also encompass consultation from the following professionals:psychologists, behavior therapists, and registered nurses.
We support individuals with Developmental Disabilities such as: intellectual disabilities, down syndrome, autism, cerebral palsy and dual diagnosis (intellectual disability and mental health problems).
Children Services
The service involves teaching children behavior management, sensory activities, social and communication skills, home skills, and leisure activities. The agency works with school personnel to assist the child to achieve his/her established IEP (individual educational plan) objectives. This program provides direct staff support in the home on an intermittent basis.
Adult Services
In the adult program the individual may be living with their family or living on their own or with a roommate. The goal is to enhance the life of the person with special needs in order to attain independence in different life skills. The Direct Support person teaches/trains the individual in the following areas: positive behavior management, hygiene/grooming, meal preparation, cleaning, laundry, self medication, social skills, money management, grocery shopping, medical appointments, etc. We also assist in applying for social security, link card, and other benefits.

Residential and Independent Living
Our Residential and Independent Living Services are designed to help adults with intellectual/Developmental Disabilities access a broad spectrum of supports so that they can lead independent lives. Our participants have different needs; thus, the resources and supports that we provide are tailored specifically for each person and last as long as necessary. When an adult seeks help from CSS, we work with them to determine a mutually agreeable level of service, using the following three categories as an entry point:
People living with their families who need to build targeted skills to live independently in their community.
People living in their own homes that require intermittent support.
People living in CSS-owned group homes in need of 24-hour assistance.
Once the level of service is ascertained, we create customized service plans which outline goals and objectives that include learning, maintaining, or increasing personal and/or household management skills. Then, we provide services in the home or in the broader community, focused on such activities as housekeeping, cooking, budgeting, socialization, or vocational skills.
Supported Employment Services
Supported employment services include customized vocational training, job-search and interviewing skills, employment support and advocacy. We work with participants to identify areas of vocational interest and establish an individualized plan to obtain, and maintain, employment in their area of interest. In addition, we help businesses in the community to understand our participants and use best practices in working with people with disabilities. Ultimately, we build bridges and long-lasting relationships between employers and adults with disabilities.
Day Program
Community Support Services’ day program – Community360 – offers adults with Intellectual/Developmental Disabilities (I/DD) activities where they choose meaningful ways to spend their day. Some activities include: structured volunteer work; creative classes in art, music and dance; activities of daily living; health and wellness classes; group exercise; classes structured around developing job skills and job searching; etc. Locations include CSS’ Family Support Center (FSC) in Cicero, CSS Academy in Brookfield, and various community locations.

Developmental Service Center
Services provided by the Developmental Service Center include case management & family support which works to ensure that consumers’ needs are identified, assessed, planned for, and monitored through a person-centered approach. Other services include an early intervention program that offers assistance to families of infants and toddlers who are experiencing or are at risk for delays in development, or have identified special needs.
Community Day Services, which assists individuals in achieving greater levels of independence and self sufficiency through individualized instruction and support in community settings, vocational training sites, and center-based activities are also provided.

Gateway Regional Medical Center Behavioral Health Services
Gateway Region Medical Center offers psychiatric care for children and adolescents,
ages six to 17 years of age through programs such as family therapy, group therapy
and individual therapy. Treatment is also offered for those living with psychiatric
geriatric services through a cognitive disorder program offering treatment for patients
aged 55 years or older experiencing symptoms of dementia, including Alzheimer’s
disease or mental impairments resulting from a stroke or physical condition. Our
therapeutic program includes education, group therapy, individual and
family counseling, medication management and recreational activities. Regularly
scheduled meetings are held with a psychiatrist and a social worker to discuss follow-
up plans and discharge arrangements. Additionally they offer in patient services including cognitive and dialectical behavior therapies, couples and family therapy,

The self-directed assistant is part of a team that works with the individual and the family to
implement an Personal Support Plan (PSP) that can be adjusted and refined according to
changing needs, goals, and wishes.
Community Day Services extends to individuals a variety of structured activities, designed to assist in achieving their maximum potential. Individuals receiving services and supports are provided instruction in self-care skills, motor skills, adult daily living skills, communication, computer training and basic academic skills. Additionally, job training is provided to enable individuals to develop and improve vocational skills and work attitude with the goal of obtaining a productive earning ability. Supported Employment assists individuals in a variety of employed environments. St. Coletta’s provides a lifelong continuum of care, beginning in early childhood. Our residential,
educational, and vocational training programs provide services for adults and children with
intellectual and Developmental Disabilities that enhance their independence and well-being.
We serve participants without regard to race, religion or economic background. St. Coletta’s meets
