Johnson and Johnson (Janssen) and Moderna are available for individuals 18 years and older.
Boosters and Additional Doses are available by appointment only.

Minors must be accompanied by parent or legal guardian at all times and Pfizer is the only vaccine manufacturer available for those aged 5-17 years old.
The parent or legal guardian must show identification.
If you do not have an email, please use COVIDvax@co.madison.il.us as your email to sign up for a vaccination appointment.
